Mangu becomes involved in a dubious, money-making scheme that requires him to lure unsuspecting volunteers for the drug trials. In the process, he convinces his parents to participate as well. However, instead of utilizing his earnings from commissions prudently, Mangu squanders them away.
Saira encounters a woman whose spouse perished as a consequence of one of the drug trials, which prompts her to raise inquiries and challenge Dr. Vivek. However, her attempts are met with resistance, and Dr. Nath dissuades her from pursuing the issue, claiming that it is futile and will be handled.
Dr. Nath and her spouse, Pratap (Ram Kapoor), endeavor to negotiate with the Chief Minister to obtain land for their project, but they face opposition. Furthermore, they are collaborating with the health minister, who seeks to replace the current CM and seize power.
The repercussions of the flawed drug become uncontrollable, prompting Dr. Vivek to meet with Dr. Nath and propose exposing the truth unless he receives remuneration to relocate abroad with his family and start afresh. However, like others who attempted to impede the drug’s advancement, he meets a similar fate, and his death is declared a consequence of driving under the influence.
Mangu’s mother falls ill due to the drug trial, and when an activist known as “OP” (Aasif Khan) visits their home, he informs them that the drug trial is most likely the cause. Mangu has been concealing the payment he received, and when his actions are exposed, he becomes irate.
Neil, Dr. Saira’s husband, is a photographer who travels extensively for assignments. However, he surprises her by arriving in Bhopal and disclosing that he will be staying for an extended period to document the suffering of the Bhopal Gas tragedy victims.
Dr. Nath and Pratap encounter significant obstacles as the CM reneges on their initial agreement, and Dr. Shindey intends to depart the hospital for their rivals. Dr. Nath reflects on her past and how her father was the only one who showed her love. After his death, she found solace in the arms of one of the servants, Roma. In the present day, Dr. Nath and Roma are conducting experiments on girls who have suffered from severe trauma, and they are also employing them as nurses.
These girls played a major role in administering several of the drug trials, and Mangu persuades one of them to assist him in obtaining the consent forms signed by his parents. As they are burdened by a loan from threatening individuals, they desire to escape their current predicament.
During a dinner invitation at Dr Nath’s house, Saira and Dr Nath share a passionate kiss which is witnessed by Saira’s husband Neil. Later, he confronts Saira about it, but she denies any wrongdoing and tries to portray herself as a victim.
With Mangu’s assistance, OP obtains further information on the drug trials and begins to build a case and raise awareness about the issue. Meanwhile, Mangu and his father are pushed to their limits by their creditors, and they violently attack one of them. They are pursued, and the chase ends at OP’s NGO, where Neil is shot and killed on the spot.

Laid bare the truth for all to see
In a flashback scene, Dr Nath is shown talking to her grandfather, who has had a stroke and is unable to move. She shows no sympathy for him and threatens to make the rest of his family suffer the way they had made her suffer before administering a lethal injection to him.
Dr Nath and Pratap are grappling with the aftermath of an IT raid at their place and are left with no choice but to seek help from the health minister, who takes advantage of their vulnerability to assert his dominance.

Saira tries to open up to her parents about her past and sexuality, but they deny and refuse to acknowledge it. Dr Nath defies the board of directors and her husband’s wishes by announcing to the staff at Manthan that she won’t step down. Meanwhile, the nurse who assisted Mangu in getting the documents, fed up with the abuse, sets fire to Roma’s house with everyone inside.
The tragic loss devastates Mangu and Dr Nath. Subsequently, Dr Nath receives a termination letter and goes to visit her aunt, who authorized the decision. She confronts her aunt, confesses to killing her own father, and then visits Saira.
Dr Nath confesses to Saira that she never loved her and only used her for her own interests. Saira condemns Dr Nath’s actions and calls her an animal, telling her that she deserves to suffer alone and powerless. Overcome with emotion, Dr Nath murders Saira and then takes her own life.
Meanwhile, Mangu infiltrates Vayu Pharma with explosives and destroys the entire building. His father is released from prison, and with the compensation provided by Saira, they are able to live happily ever after. This brings a momentary victory for the common people.
